A Medium of Instruction certificate is a letter from your university stating that your degree was taught and examined in English. It is issued by your institution, not by a testing body, and there is no fee to a third party.
In practice you request it from your registrar or examination department. Some universities have a standard form and issue it in days. Others have never been asked and take weeks to produce something, which is the main reason to request it early rather than when a deadline is close.
A useful certificate names you, names your degree and the years you studied, and states plainly that the medium of instruction and examination was English. One that merely says the university is an English-medium institution in general is weaker and may not be accepted.
